Please complete this 2011 Pool Authorization form. This form must be received by pool attendant before anyone in the household will be issued an ID card or permitted to use the pool facilities. ID cards are only issued to authorized family members 14 years of age and older. CHILDREN 13 YEARS OF AGE AND UNDER MUST BE SIGNED IN AND ACCOMPANIED AT ALL TIMES BY AN AUTHORIZED ADULT FAMILY MEMBER! NO EXCEPTIONS!
